**You can download app "scanner pro" for free on your phone, take a picture of the forms save as PDF & email. 


Each player attending will need to submit registration and payment by Friday, June 10.
  1. ​Completed Registration Form (NOTE: By signing you agree that you've read the Concussion      Form)
  2. Copy of Current Physical (if physical is current, we will pull copies from the school) 
  3. Payments $165 (Payments can be made via check payable to Cougar Football Club PO BOX 42 Bothell, WA 98041 or thru PayPal link below.

Players that worked the Mattress Fundraiser were asked to sign-in. Those that did will receive a $30 credit towards Camp fee. Please Check list to know if your player gets the credit and choose appropriate option when paying via PayPal link.
